This is to verify that I, ... give permission for the Department of Local Government, Sport and Cultural Industries (DLGSC) and the State of Western Australia (the State) to take and use any or all of the following materials:

1. photographic images of me and/or my pet;
2. video recordings of me and/or my pet;
3. films of me and/or my pet; and
4. audio recordings of me and/or my pet,

on the following terms:

1. I irrevocably authorise DLGSC and the State to edit, alter, copy, exhibit, publish and distribute the materials in print and electronic format, including in printed and electronic publications, flyers, posters, online, social media, television broadcasts and radio broadcasts for the purpose of promoting programs, services and functions performed by DLGSC and/or the State.
2. The materials become the property of DLGSC and the State and will not be returned.
3. I will not have the right to inspect or approve the materials or finished product.
4. There will be no restrictions on the number of times the materials are used.
5. This is a free permission to DLGSC and the State. I am not entitled to any compensation or payment for the taking or use of the materials as set out in this authority.
6. DLGSC and State will not sell the materials, whether as a standalone file or otherwise, to any third parties.
